Instructions: Please read carefully:

You will need to register twice to play inline hockey with HIHA.

First, you will need to be register with USA Hockey as an Inline Player for the 2009-2010 season.

If you registered with USAHockey after July 31, 2009, then go to www.usahockeyregistration.com/. Select Request an additional conformation page, follow the instructions, and print the final conformation. USAHockey does not charge a fee to send you an additional conformation.

If you have NOT registered with USAHockey since August 1, 2009, then go to www.usahockeyregistration.com/. Register as an Inline Player for the 2009-2010 season. You will have to do this even if you have already registered with USAHockey as an ice player. USAHockey charges $30 to register.

Second, you will need to register with HIHA. The fee and form are included below. You can either mail this form in, along with the USAHockey registration conformation to us, or bring it to Play-It-Again Sports on January 23, January 30, or February 6, 10:00 a.m. - 1:00 p.m.

Complete a separate form for each child and mail to:
